Dorset CCG wants to hear from children and young people aged 25 and under about their experiences of accessing audiology services.

They have launched a survey that young people and their parents/carers are invited to complete here:

This survey closes on March 12th 2021.

Please note that due to data protection laws, you need to be 16 years and above to fill in the survey. Parents and carers are able to fill in the survey on behalf of under 16s.
